The confidentiality/disclosure form is your acceptance as well as your acknowledgment of our role in the process.  

The second form is also an acknowledgment of our role as a NONREPRESENTATION BROKER. 

A copy of your driver’s license is required to verify signatures. 

Upon receipt of both executed forms to our office, I will see that a short summary of the business is e-mailed or faxed back to you. Thanks again.
